Unable to add columns to table extended attributes v3.1.0.034 64-Bit

I know this problem has been reported before but none of the other posts seem to have a resolution. When I create a new extended attribute with a table type, I can add the table name and description but I am unable to add the column name and description.

It seems like the form might not be displaying correctly, it's as if the buttons have disappeared from the form and I am unable to scroll down to them. (see attached).
